  • \n\t\t\t\t\t<\/span>\n\t\t\t\t\tPrescription - the medical recommendation for a wheelchair, in original;<\/span>\n\t\t\t\t\t\t\t\t\t<\/li>\n\t\t\t\t\t\t\t\t\t\t\t
  • \n\t\t\t\t\t<\/span>\n\t\t\t\t\tCopy of identity card \/ birth certificate (for children); <\/span>\n\t\t\t\t\t\t\t\t\t<\/li>\n\t\t\t\t\t\t\t\t\t\t\t
